    Bilateral mammogram

    The breasts are heterogeneously dense with increased density at the site of fthe clinical lump upper and oouter quadrant right breast. No focal mass, suspicious microcalciflications or architectural distortion. Small cluster of a few benign microcalcifications right breast at 9 o'clock. The axilla are clear.

    Bilateral breast Ultrasound

    The clinical lump within the right breast at 9 o'clock 4 cm from nipple corresponds to a irregular area of heterogeneous hypoechoic sofft tissue density with mild vascularity measuring 25x17x19 mm. There are at least 4 other ffocal hypoechoic nodular areas off soft tissue density between 9 o'clock ans 12 o'clock, the largest at 12 o'clock, the largest at  12 o'clock mesuring up to 7 mm.

    In the left breast a irregular area of hypoechoic sofft tissue density at 4 o'clock 4 com from the nipple with mild vascularity measure 15x8x13 mm. 3 mm cyst at 3 o'clock 2 cm from the nipple.

    the Right axilla is clear. In the left axilla there are 2 lymph nodes of similar size 20 mm with normal hila and asymmetric thickend cortex measuring up to 6 mm. A 9 mm rounded lymph node is uniformly hypoechoic with no echogenic hilum.

    Conclusion

    Bilateral suspicious breast lesions with possible multifocal tumour right breast. Atypical left axillary lymph nodes possibly metastatic. Ultrasound guided core biopsy of the largest breast lesions and FNA left axilla suggested.
