    Hello

Heart falilure is the condition of heart that fail to pump enough blood to meet the body's needs for blood and oxygen.

The most often symptom of heart failure including shortness of breathing, chronic cough or wheezing, tired or fatique, fluid accumucation or edema such as legs swelling.

Heart failure can also result in scrotal edema. If the circulatory system is not moving fluid throughout the body, edema occurs. The kidneys again begin to retain sodium and water resulting in ascites in the peritoneum. This edema can often spread to the lower extremities. In severe cases of heart disease, anasarca (or generalized edema) can include scrotal edema so the difference serverity effect the difference fluid accumulation indicate that your friend may have more severity.

then if my friend can go back to normal?

It's depend  on the cause of his heart failure. If the cause can treatable with specific treatmen he will become normal. However he should close follow up with cardiologist regullary.
